Recommended fields
- Full legal name and any other names used
- Date of birth, address, phone, and email
- Preferred contact method, and whether it's safe to leave messages
- Type of legal matter (family / criminal / civil / estate / business / immigration / other)
- Brief description of the matter (2–3 sentences)
- Names of all other parties involved (individuals and companies) — used for our conflict check
- Are there any upcoming deadlines, court dates, or statutes of limitation you're aware of?
- Have you consulted or retained another attorney on this matter?
- How were you referred to our firm?
- Emergency contact
- Signature and date
Tips for this template
Opposing parties first
The conflict-check question is why this form exists. Ask for every individual and business name involved, including alternate spellings.
Flag deadlines prominently
A missed statute of limitations discovered late is a malpractice risk. Make the deadline question impossible to skip on the printed form.
Keep case detail minimal at intake
Two or three sentences is enough to route the matter. Detailed facts belong in the privileged consultation, not on a form filled out in a waiting room.
